a) Terms and conditions should be prepared:

• Copy of Building Assets Tax (PBB) of the latest year or authorization from the district (Kelurahan).
• Copy of Identity Card (KTP) & Family Card (KK)
• Authorization Permit for Individual Business (SIUP) – for commercial customers
• Authorization from the respected Institutions/Department (for religious building or social foundation customers)

b) There are 2 things that should be followed:

1. Contact PALYJA’s 24 hours Call Center
   • Prepared all the documents stated in point “a”.
   • PALYJA’s officer will contact/visit you to conduct survey to the location and collect the conditions documents prepared.
   • During the visits’ of PALYJA’s officer, signed the Acceptance and Agreement Letter that has been prepared.
   • Within 5 working days after the terms and conditions has been completed, our officer will install piping network and water meter in your premises (should there be an available network in your locations).
2. Visiting the nearest Customers’ Relations Office (KHP)
   • Bring along the documents stated in point “a”.
   • Filled up the application form available in KHP office.
   • PALYJA’s officer will conduct survey to your location.
   • During the visits’ of PALYJA’s officer, signed the Acceptance and Agreement Letter that has been prepared.
   • Within 5 working days after the terms and conditions has been completed, our officer will install piping network and water meter in your premises (should there be an available network in your locations).

Tariff Classifications are base on:

a. Building purposes
(whether it is for business or household purposes in the public housings/real estate).
b. Building size
